## About
Bennett Integrative Health is a mostly virtual concierge practice led by Roxanne Bennett, DNP, NP-C, a board-certified Adult-Gerontology Primary Care Nurse Practitioner (DNP and MSN, University of Alabama at Birmingham; Certified Health Coach, American Council on Exercise; Associate of the American College of Cardiology). It offers primary care, medical weight loss including GLP-1 care, hormone therapy, heart health and ultrasound screenings, genetic testing, health coaching and wellness services, with longer visits and direct messaging. It is a teaching practice that precepts nurse practitioner students. It is self-pay (no insurance billing for visits) and accepts HSA and FSA cards. Patients are ages 13 and up.

Monday to Friday, 8:00 a.m. to 5:00 p.m., and Saturday, 8:00 a.m. to noon, by appointment only (no walk-ins). Monday and Saturday are virtual visits only; Tuesday to Friday, visits are virtual or in person.

## How to start (effective November 1, 2026)
- Focused Assessment: $249. Best for one main goal, like everyday primary care or weight management.
- Comprehensive Assessment: $399. Best for several concerns, hormone questions or a full whole-person look.
- A discovery call is available for anyone unsure which assessment fits.

## Memberships (optional, after an assessment)
- Essential Primary Care: $90 a month or $990 a year
- Active Weight Management: $90 a month or $990 a year
- Weight Maintenance: $60 a month or $660 a year
- Comprehensive Care / HRT: $150 a month or $1,650 a year
- Every membership includes unlimited secure messaging with the provider and same or next-day visits. Pay-as-you-go visits are also available.
- All members get 20% off office visits, ultrasound screenings, IV therapy and add-on services.
- Families: Ask us about a customized family membership. Adding members of the same household to your plan comes with a discount.

### Primary & Preventive Care
- Primary Care: Routine and sick visits, annual labs, prevention and chronic condition management, virtual or in person.
- Rapid Molecular Testing: Molecular testing for COVID-19, flu, RSV and strep, right here in our Ringgold office on our ID NOW analyzer. Skip the urgent care clinic.
- Prescriptions & Medication Review: A careful look at everything you take: what is helping, what may be unnecessary, and what may be causing side effects. Roxanne works with your other prescribers as your advocate.
- Complex Care Coordination: Record review after a hospital stay or specialist visit, with care coordination and a clear follow-up plan, so nothing falls through the cracks.
- Home Visits: If you can't come to us, we may be able to come to you. Home visits are available when appropriate, within a set distance of the office.
### Weight & Metabolic Health
- Medical Weight Loss: An individualized, medically guided plan, including GLP-1 medications when appropriate. Roxanne prescribes and helps you find the best price on compounded or FDA-approved options, with a focus on protecting muscle.
- Fit3D Body Scans: A full 3D body scan in seconds: body fat, lean muscle, measurements from neck to calves and posture analysis, so you can see real progress beyond the scale.
- Health Coaching: Certified health coaching with a nutrition and exercise plan and ongoing support, virtual or in office. It is for anyone who wants more energy, less stress and better long-term health.
### Hormone Therapy
- Hormone Therapy: Evaluation and treatment for hot flashes, low libido, fatigue, weight gain, poor sleep, acne and other signs of hormone imbalance, through perimenopause, menopause and beyond. Hormone pellets are available for women and men.
### Heart Health & Screenings
- Heart Health: Cardiovascular risk reduction, including care for high blood pressure, high cholesterol, diabetes and weight, plus smoking cessation support and a 6-lead EKG.
- Ultrasound Screenings: Thyroid, kidney, pelvic, and liver and gallbladder screenings, plus a cardiovascular package with Chad Bennett, who has more than 20 years in ultrasound and diagnostic imaging.
- Genetic Testing: Testing that makes your care more personal, from how your body handles medications to the heart risks that run in your family. Genetic testing starts at $350.
### Aesthetics & Wellness
- Wellness Injections & IV Hydration: B-12, glutathione, lipotropic, sermorelin and NAD+ injections to support energy, metabolism and recovery, plus IV hydration.
- Dysport & Sclerotherapy: Dysport to temporarily soften moderate to severe frown lines, and Asclera sclerotherapy, an FDA-approved treatment for spider veins and small varicose veins of the legs.
